Aim Home Health’s Medical Social Workers form part of our multi-disciplinary staff. Their role is to assess the social needs of the patient and also to extend such services to family members. When it comes to disease, old age or injury, we believe that it’s no longer just the patient who is affected but the whole family becomes involved. Medical Social Workers can assist the entire family and will be able to recommend community services to ease the difficulties they are going through.
